The patella is supposed to align within the specified location and move smoothly as the knee is bent or straightened. There are different conditions, including injury, that can result in malpositioning of the kneecap, resulting in complete dislocation or subluxation. Depending on the age of the patient and other factors such as lifestyle, the surgeon will consider the best surgical option once the non-operative measures have failed. This could include soft tissue procedures such as MPFL (Medial Patellofemoral Ligament) reconstruction, cutting bones (ostotomy) to realign the patella movement trajectory, or combining both types of procedures to achieve appropriate alignment and function.

What to expect at the time of your surgery:

Before the surgery, anaesthesia is administered to ensure the patient’s comfort. It can be either regional or general anaesthesia. Depending on the decision of bone or soft tissue correction, incisions can be small if the procedure is done arthroscopically or longer if bone cuts are required to realign the patella. The MPFL is an important ligament on the inner side of the kneecap that gets torn as the patella dislocates outward. A graft, either from the patient or a donor, can be used, as well as synthetic grafts, to reconstruct the torn ligament. The surgeon will assess the knee joint by introducing an arthroscope and may require lateral retinacular release to help improve the overall position of the kneecap. Small incisions are required to create a bone tunnel in the thigh bone as well as the patella. In certain cases, depending on the patient’s age and the severity of the problem, bone cuts are required to achieve satisfactory alignment. These can be in the thigh bone (trochlea) or the shin bone (tibia).  After completing the procedure, the incisions are closed with sutures or adhesive strips. Sterile dressings or bandages are applied to the incision site. The patient is typically monitored for a short period before being discharged, and post-operative instructions for pain management, rehabilitation exercises, and recovery are provided. 

What to expect after your surgery:

You will be provided with clear post-operative instructions, including pain management. A knee brace application is expected for the initial few weeks to provide support and protect the reconstruction. At two weeks, the sutures will be trimmed, and a wound check will be performed. Physiotherapy will be required gradually to achieve the full range of movement and strength of the muscles. Driving and office work may be possible in 8 weeks, depending on the overall recovery and the type of procedure performed.
